Community Re-entry and Re-education
Recovering from a devastating illness or injury not only takes time, but it often times removes you from your circle of friends and family. And, sometimes when you get back home, things are different than they used to be.
Our hospital offers a comprehensive community re-entry program to help patients transition back home and to living an active lifestyle. We offer different support groups so patients can talk to others that have been in similar situations.
Our case managers also work with patients and their family to help arrange for adaptive equipment and home health assistance if it’s needed.
